For Referring Providers
We value our partnership with referring clinicians and are dedicated to providing your patients with exceptional surgical care and a seamless transition back to your practice. Below is the contact information for your team to coordinate care effectively.
Send referrals & records
Please send all patient records, imaging reports, and insurance information via secure portal or fax to ensure timely review of your clinical requests.
What to include with your referral
- Patient demographic information (including address and contact numbers)
- Copy of insurance card(s) (front and back)
- Recent office notes related to the surgical consult
- Relevant imaging reports (CT, MRI, Ultrasound, PET scan)
- Related pathology reports
- Recent laboratory results (CBC, CMP, CEA, etc.)
- Previous surgical history / operative reports
Outside imaging
If imaging was performed at an outside facility, please ensure the patient brings a disc of the images or that they have been uploaded to the Freeman Health System PACS system.
Time-sensitive referrals
For urgent surgical consultations or suspected malignancies, please contact our office directly by phone to expedite the referral process.
